Application and Forms

One time volunteer opportunities can be scheduled by calling Tammy at 269-7506 . All one-time volunteers need to sign a one-time Volunteer Contract (pdf).

Ongoing volunteers need to complete a Volunteer Packet (pdf), including a Background Screening Request, application, code of conduct and core DYS policies, before volunteering.

All ongoing volunteers need to attend an orientation, which is always scheduled on the first Thursday of the month at 4 to 6 p.m. in the Youth Services Conference Room
